Puppy Purchase Contract

I understand that this puppy is in good health and has been wormed according to vet recommendation. I also understand that this puppy will have received the first vaccination at 6 weeks and will need to continue with the vet recommended vaccination schedule depending upon age at the time of purchase.

I understand that if my vet detects symptoms of any preexisting illness within 72 hours from purchase date, I may return the puppy with documentation from my vet, and be refunded the full purchase price.
